Intravenous regional anesthesia involves the intravenous injection of local anesthetic distal to an occlusive tourniquet. It is often referred to as a Bier block since the first documented use was by August Bier in 1908. Although it is most commonly employed for surgery on the upper extremity, it has been done on the lower extremity as well.

Bier was known for his work with spinal anesthetics, and this technique was a natural offspring of his work with tourniquets and local anesthetics. The technique of exsanguinating an upper extremity and injecting local an … August Karl Gustav Bier (1861–1949) In the late nineteenth century, August Karl Gustav Bier was a surgeon in Germany who studied spinal cord anesthesia, later called spinal block.

The local anaesthetic diffuses into the surrounding nerves. Differential diagnosis can be aided by selective anesthesia of the nerves of the digit. The diagrams on this page illustrate the sites for four regional nerve blocks in the front and hind feet.

Body position: Intravenous Regional Anesthesia (IVRA).
The Bier block technique provides analgesia, muscle relaxation and a relatively bloodless field. It involves injection of local anaesthetic into the venous system of an extremity which has previously been exsanguinated by gravity or compression.

Bier determined through cadaveric studies that the mechanism of action of IVRA involved both direct local anesthesia bathing local nerve endings and indirect local anesthesia through conduction block, probably due to local anesthetic transported to the substance of the nerves via the vasa nervosa.
Bier block is a valuable technique for short procedures, but as the local anesthetic is being injected intravenously, it is important to make sure that the tourniquet is not released for at least forty five minutes which will give local anesthetics enough time to be absorbed slowly and to prevent any systemic toxicity. In 1908, Bier pioneered the use of intravenous regional anesthesia, a technique which is commonly referred to as a "Bier block". This technique is frequently used for operations of brief duration upon the hand, wrist, and forearm. It can also be used for operations of brief duration upon the foot, ankle, and leg. Bier Block – Local anesthetics are injected intravenously to numb a limb, typically the arm, and then a tourniquet is applied to prevent the anesthetic from leaving the area.

The Bier block is disadvantageous 18 Jul 2018 Background. Intravenous regional anesthesia (IVRA) or Bier Block is a simple and effective but underused anesthetic technique for hand and Intravenous regional anesthesia (IVRA) of the limb was first described by Bier in. 1908.1 The original technique involved the surgical exposure of, and direct 14 Oct 2020 All complications associated with IVRA were reviewed. The lowest dose of local anesthetic associated with a seizure was 1.4 mg/kg for lidocaine; A Bier block essentially consists of injecting local anesthetic solutions into the venous system of an upper or lower extremity that has been exsanguinated by 24 Oct 2011 Department of Anaesthesia, Dayanand Medical College and intravenous regional anesthesia (IVRA), also called Bier's block, was first August Bier introduced this block in 1908.
A Bier block is performed by inserting a small gauge IV as distal in the operative extremity as possible. Use of Bier Block (Intravenous Regional Anesthesia) for reduction of distal radius fracture.
